Methods used to coordinate services

Care coordination is a client-centered, assessment-based, interdisciplinary approach to integrating health care and psychosocial support services in which a care coordinator develops and implements a comprehensive care plan that addresses the client’s needs, strengths, and goals.

1. service identification and coordination

  • determining the least restrictive environment for client safety
  • identifying client specific community support systems
  • locating short term and long term placements
  • evaluation and determine need for additional services
  •  facilitating client transition, assessing the suitability of the caregiver/home/placement/transitional facility, evaluate client's ability to access treatment and support services,
  •  identify social services that meets a clients basic needs and incorporating community support system for transient and homeless client.


2. Providing advocacy and support :

  •  methods to increase clients ability to advocate for oneself and access needed resources, 
  • knowledge of housing, employment and equal opportunity regulations that protect the clients rights, 
  • evaluating the safety of clients placement and regulations related to residential placement


Resources to keep in mind in assisting diverse population

  • does it provide for the overall well being of the client- holistic perspective (physical, psychological, emotional, cognitive needs, social needs, medical needs, culture and language oriented. does the activities suitable for client
  • diversity not affecting the service
  • based on the nature and situation of client, in terms of better problems, having insight into problem, and based on which the type of service is based
  • access  to resources for financially challenged like county welfare, food banks, housing assistance or public assistance programs, local parish soup kitchen
  • services for ethnic population available like Jewish family services, black resource center
  • additional protection needed for children, high risk youth and elder. resources for these population include big brother/big sister, children of the knight for children engaging in prostitution, youth shelter, senior citizen center, adult day care center, hospice and Alzheimer association.
  • gender specific support centers or resources include Veterans association, battered women s group
  • gay and lesbian population are another group who in need of services at various levels. gay and lesbian community service centers,national gay and lesbian task force.
  • if the client seems to be philosophical, spiritual or religious, clear the stand before providing resources related to the same like spiritual counselling, charity associations , parish center
  • educational service may benefit a client much better like adult schools, esl classes, vocational training, parenting class, educational therapy
  • housing services for elderly include senior citizen apartment, senior centers for peer group support, meals on wheels for delivering food to housebound adults, assisted living facilities (meals and medications, social activities etc), skilled nursing facilities (for citizens for severe physical and cognitive impairment), hospice for seniors who are terminally ill whose death is expected to be within 6 months time. 
  • resources for unemployed include job coaching, sheltered workshop and supportive employment
  • SSI is another welfare program 
  • food stamps, medicare or medicaid 
  • supportive housing program, shelter, single room occupancy program, transitional housing, safe heaven supportive housing program.

in coordinating resources one must begin with knowing the community resources, assess care giver, assess care/ placement center, assist in transition, evaluate the placement, knowledge of laws/rules/statues of federal and state governments.
