relevant when diagnosing a client. When
diagnosing a client using the DSM-IV, the social worker should consider the
individual’s clinical symptoms, psychosocial stressors, and social and physical
functioning.
(77). D. The child was referred to the social
worker for therapeutic services. Answers
A, B and C should be done by the child’s juvenile officer. Should the social worker engage in any of the
activities described in Answers B and C, the Social Worker/Client relationship
and responsibility to maintain confidentiality would be tarnished.
(78). B. In this situation, because of the short
length of time that the client has been in therapy and because the client did
not give a reason for wanting to discontinue treatment, it is appropriate for
the social worker to explore with client her reasons for wan ting to
terminate. Often times, when a client
begins to approach a difficult, painful and anxiety-provoking issue, the client
would rather avoid the issue. If this is
the case, it is appropriate for the social worker to bring this to the client’s
attention; however, the social worker should never force or try to “convince”
the client to continue with treatment.
If the client provides a reason, e.g., job relocation, the social worker
should begin the termination process.
(79) B. With the exception of your supervisor
or colleague or under court order, a social worker should not discuss a client
with anyone, including spouses and other family members. Answer A, accepting a token gift over the
holiday season would not be a breach of the code of ethics; however, social
workers should not accept expensive or otherwise inappropriate gifts from clients
at any time. Answer C, reducing your
fees to accommodate a client’s financial situation is perfectly acceptable and,
at times, advisable. Answer D,
commenting on a client’s physical appearance as part of the treatment process,
e.g., I see that you look tired, is acceptable.
On the other hand, commenting on a client’s physical appearance, i.e.,
You look nice today, is not acceptable.
(80) A. Denial is a defense mechanism in which
an individual refuses to believe or accept the reality of a situation in order
to avoid the accompanying painful memories and thoughts.
(81). B. The Psychoanalytic Theory focuses on
the past. Freud, the father of
Psychoanalytic Theory, believed that most behaviors and thoughts are driven by
unconscious, repressed memories from childhood.
(82). B. Projective Identification is the
unconscious modeling of one’s self upon another individual’s actions, thoughts
or behaviors.
(83). C. Magical Thinking is when a person
irrationally believes that her/his thoughts, actions or desires have influenced
or cause certain events to occur. This
is often seen in children and in individuals diagnosed with schizophrenia.
(84). A. When working with a truant child, it is
most important to look at the child’s peer relationships. It is also of important to look at the
child’s familial relationships.
(85). B. In this scenario, the client meets the
criteria for depression. Elavil is a
drug that is commonly used to treat depression.
(86). A. Echolalia is often seen in individuals
with Tourette’s Disorder. Echolalia is
the repetitive imitation of the speech of another. This is normal for infants under the age of 1
year. It is often seen in adult
individuals diagnosed with schizophrenia.
(87). B. Paradoxical Intervention is often used
in family or couple’s therapy. It is a
treatment approach where the social worker tells the individual to
intentionally continue the “problem” behavior or action, e.g., fighting, and to
improve on it. This makes the individual
aware of the existence of the behavior and allows him/her to control it. Answer A, Reverse Psychology is when a
certain thought or image is projected onto an individual in efforts to assist
the individual in understanding the thought or image as the opposite. Answer C, Behavioral intervention is a broad
term used to describe methods of intervention to change one’s behaviors. Answer D, Psychodrama is a treatment
technique often used in group therapy where the clients are directed to perform
roles playing ei ther themselves or another person (usually their
antagonist). This allows individuals to
release anxiety, practice handling the situation better, and view the situation
from the other person’s behavior.
(88). B. The problem solving model involves a series of interactions between the client system and the
practitioner, involving integration of feeling, thinking and doing, guided by a purpose
and directed toward achieving an agreed-upon goal".
(89) A. Modeling is a behavioral learning
technique where a person is asked to imitate one or more person’s behaviors.
(90). C. Poor hygiene is generally a sign of
neglect. Answers A, B and D are all
signs to look for when physical abuse is suspected.
(91). C. Physical and Medical problems left
untreated generally is a sign of neglect.
Answers A, B and D are all signs to look for when sexual abuse is
suspected.
(92). B. According to the Psychosocial Theory,
Catharsis is the key trigger to change.
Catharsis is the process of “release” by the client that relieves
anxiety. Do not get confused with
Cathexis (Answer C). Cathexis refers to
the concentration of emotional energy onto a person, idea or object.
(93). A. The social worker’s responsibility as a
therapist is to assist clients in resolving problems on their own. In this
situation, the best response would be for the social worker to explore with the
client the reasons that she is confused in efforts to assist the client in
coming to a decision herself. Answer B
is correct to the extent that a social worker should never project her/his
personal feelings and thoughts onto the client.
However, telling the client that you cannot give her direct advice may
have negative implications on the social worker/client relationship. Answer C is not appropriate and may also have
negative implications on the social worker/client relationship. Answer D may be appropriate after the social
worker explores the client’s feelings about her marriage.
(94). C. Avoidance is a defense mechanism,
similar to Denial, when an individual refuses to face certain situations
because they present unconscious impulses or punishments. Answer A, Diversion is not a recognized
defense mechanism. Answer B,
Displacement is a defense mechanism where an individual transfers certain
thoughts, feelings or wishes to reduce anxiety.
Answer D, Resistance is not a defense mechanism. Resistance is a therapeutic concept referring
to an individual who stops working through her/his issues.
(95). A. In this situation, the social worker
should explain the boundaries that he sets on the therapeutic relationship with
all clients. Answers B, C and D are all
inappropriate responses to a client.
(96). C. When a client presents with a dual
diagnosis, one of which is substance abuse disorder, the social worker should
first treat the client for substance abuse.
A client needs to be clearly thinking and not intoxicated in order to
undergo treatment for the other disorder.
(97). A. Congenital
is a term that is used to indicate that a disorder or condition was present at
birth. Such condition is not necessarily
inherited (Answer B), temporary (Answer C), or permanent (Answer D).
(98). B. A Bureaucratic agency exhibits
“vertical” power and control. That is,
power and authority runs from the top (senior level management or board of
directors). Down (employees and staff).
In a bureaucratic agency, departments are clearly defined and employees
are placed within each department. Each
department and employee has specific tasks and functions. In addition, communication is often times
more formal.
(99). D. Role Reversal is when a person changes
her/his behavior and begins to act in a way that is expected of another person.
(100). B. The Social Worker should acknowledge
the client’s distress and empathize with the client first. The client needs to feel that she is being
heard. After the client’s feelings are
acknowledged, the Social Worker should explore with the client her feelings of
distress and anger (Answer C). Answer A
and D may be appropriate at some point in time.
